Jacob Hardt, Certified Intervention Professional

My background

I know how hard change can be — and how much becomes possible when the right person stands beside you.

I am a Certified Intervention Professional (CIP) with ten years of experience guiding individuals and families toward meaningful change. My work grew from lived experience: I faced addiction myself, rebuilt my life, and learned that lasting change begins with honesty, connection, and hope rather than shame.

Today I support people through addiction, depression, behavioral struggles, family crises, and major life transitions. I also coach people who are already doing well but want more direction, accountability, or a way to become their best selves. Help should not carry a stigma, and no one needs to wait for rock bottom before asking for it.

I am based in New York City, where my home and roots are, and I work throughout Kansas City, where I have built deep connections. I work with clients and families nationwide, meeting each situation with discretion and a plan shaped around the person rather than a formula.

On select cases, I collaborate with my mentor and colleague Brad Lamm through Intervention.com and BreakFree Journeys. Those experiences continue to strengthen my commitment to thoughtful, family-centered care.

My mission

Make exceptional support feel human, reachable, and free of stigma.

Evolve exists to help people see a future beyond the moment they are in. Whether the need is urgent or the goal is personal growth, my mission is to create clarity, restore connection, and help each person take the next honest step.

Dignity first

Every person deserves to be heard without judgment, labels, or shame.

A practical path

We turn fear and uncertainty into clear choices and manageable next steps.

Trust and discretion

Families share deeply personal moments with me, and I protect that trust.

“I am not what happened to me. I am what I choose to become.”

— Carl Jung
